Job summary

Bristol Hospice in the United States is seeking a compassionate Nurse Practitioner to provide medical care and palliate and manage terminal illness under physician supervision, serving adults and children.

You will complete hospice face-to-face visits, coordinate with the interdisciplinary team, develop plans of care, and support patients and families with end-of-life counseling. A Master's in Nursing and an NP license are required; hospice experience preferred.

Qualifications

• Must have a master's degree in nursing and an accredited NP program.
• Current nursing licensure and NP licensure in the state of practice are required.
• Certification in Hospice and Palliative Nursing (CHPN) is preferred.

Responsibilities

• Provide medical care and palliate and manage terminal illness under the supervision of a collaborating physician.
• Complete hospice face-to-face visits for eligibility assessments during hospice benefit periods.
• Review, coordinate, and oversee patient care with the hospice interdisciplinary team and other practitioners.
• Counsel patient and family on end-of-life issues and support patient-centered goals.
