Job summary

Buena Vista Hospice is seeking a compassionate Hospice Nurse Practitioner to deliver high-quality end-of-life care across Ventura and Los Angeles Counties. The role involves patient assessments, symptom management, hospice eligibility determinations, and collaborative care planning with an interdisciplinary team.

The ideal candidate holds an active NP license with a Master's in Nursing, at least 1 year of NP experience, and preferably hospice/palliative care exposure.

Qualifications

• Active and unrestricted NP license.
• Master's degree in Nursing from an accredited program.
• Minimum 1 year of Nurse Practitioner experience.
• Minimum 1 year of hospice or palliative care experience preferred.
• Valid driver's license, reliable transportation, and auto insurance.

Responsibilities

• Conduct comprehensive patient assessments, including history and physical exams.
• Perform hospice eligibility evaluations and face-to-face visits.
• Develop and manage individualized treatment plans.
• Prescribe medications and treatments per state regulations.
• Collaborate with physicians, nurses, social workers, and other healthcare professionals to coordinate patient care.
• Manage pain and symptom control to improve comfort and quality of life.
• Educate patients and families on disease progression and end-of-life care.
• Complete timely and accurate clinical documentation.
• Ensure compliance with Medicare, Medicaid, HIPAA, and hospice regulations.
