Job summary

Vitas Healthcare Corporation in Sacramento, CA is seeking a licensed Nurse Practitioner for a hospice care role. You will assess patients, develop care plans, and coordinate with physicians and the care team to ensure compassionate end-of-life support.

The position requires NP licensure, completion of an accredited NP program, and at least 2 years of hospice/home health experience. You will work closely with hospice staff and volunteers as part of an interdisciplinary team.

Qualifications

• Minimum 2 years of hospice, home health, or community health experience in the past 5 years.
• Lifting and transferring patients as needed; lifts over 20 pounds.
• Required vision: close vision, distance vision and ability to adjust focus.
• Ability to stand or walk over one hour per day; talk, hear, sit, stoop, kneel and crouch.

Responsibilities

• Records physical findings, and formulates plan and prognosis, based on patient's condition.
• Discusses case with physician and other health professionals to prepare comprehensive patient care plan.
• Prescribes or recommends drugs or other forms of treatment.
• Monitors and maintains progress of patients and reports progress to supervising physician.
• Has the ability to work collaboratively with hospice employees and volunteers as part of an IDT.
