Job summary

Legacy Hospice, Inc. in Mississippi is seeking a Nurse Practitioner to provide compassionate, patient-centered hospice care in patients' homes within the community.

You will perform eligibility assessments, comprehensive evaluations, and develop care plans while collaborating with the interdisciplinary team to ensure high-quality symptom management and timely interventions. This role requires an APRN license, MSN, hospice/geriatric certification preferred, and at least 1 year of hospice or home

Qualifications

• Licensed APRN in the state of employment.
• MSN or equivalent nursing degree.
• Hospice, palliative care, or geriatrics certification preferred.
• Minimum 1 year of hospice or home health experience.
• Current BLS/CPR certification.
• Valid driver's license and reliable transportation.
• Comfort with mobile tech and electronic charting.
• Ability to work independently and manage time effectively.
• Compassionate bedside manner; professional conduct.
• Pre-employment drug screening including marijuana.

Responsibilities

• Conduct face-to-face hospice eligibility assessments.
• Perform comprehensive patient evaluations, histories, exams, and care coordination.
• Develop and update individualized hospice care plans.
• Collaborate with medical director and interdisciplinary team.
• Prescribe medications and treatments as indicated.
• Educate patients and caregivers on symptoms, progression, and planning.
• Participate in quality improvement and compliance initiatives.
• Maintain accurate clinical documentation.
