Job summary

Bristol Hospice - Pathways (Waxahachie), TX is seeking a compassionate Nurse Practitioner to join our hospice team. You will provide medical care, palliate and manage terminal illness under the supervision of a collaborating physician, caring for adults and children with dignity and comfort.

Requirements include a master's degree and Texas NP licensure with an accredited NP program, board certification in Hospice/Palliative care preferred, and prior hospice experience.

Qualifications

• Master's degree with NP licensure in Texas.
• Current nursing licensure in Texas.
• Accredited Nurse Practitioner program.
• Certification from an approved certifying body for advanced practice nursing; Hospice/Palliative preferred.
• Experience in hospice or palliative care.

Responsibilities

• Provide medical care, palliate and manage terminal illness under the supervision of a collaborating physician, caring for adults and children.
• Complete history and physical examinations, evaluate health, order/interpret tests, document findings.
• Develop individualized care plans, prescribe treatments, coordinate hospice services.
• Complete hospice face to face visit requirements for eligibility.
• Provide clinical information to the hospice medical director for interpretation of eligibility.
• Collaborate with the interdisciplinary team to create, review and revise the plan of care.
• Communicate with patients and families about end-of-life issues.
