Job summary

Bristol Hospice in Georgia is seeking a compassionate Nurse Practitioner to join our interdisciplinary team and deliver high-quality hospice care to adults and children. You will assess, plan, and manage terminal illnesses with physician supervision, focusing on comfort, dignity, and family support.

The ideal candidate holds a Master’s degree in Nursing, an active NP license, and hospice experience. Certification in a specialized area is preferred.

Qualifications

• Must be a graduate of an accredited School of Nursing and accredited Nurse Practitioner Program.
• Must have current nursing licensure in State.
• Must have a master's degree with a minimum of one (1) year Nurse Practitioner experience and a minimum of one (1) year hospice or palliative care experience.
• Must have accreditation from an approved certifying body for advanced practice nursing as required by the State.
• Certification in a specialist area preferred; (e.g. Hospice and Palliative Nursing (CHPN))

Responsibilities

• Provide medical care, palliate and manage terminal illness under physician supervision.
• Complete history and physical examinations and document findings.
• Develop individualized plans of care and coordinate hospice services.
• Ensure hospice eligibility through face-to-face visits and local determinations.
• Collaborate with the hospice interdisciplinary team to manage symptoms and care plans.
