Job summary

Heart'n Soul Hospice in Memphis, TN is seeking a Nurse Practitioner to join our team delivering compassionate palliative care. You will consult with patients and families, educate them on disease processes and end-of-life care, and collaborate with the IDG to develop care plans.

The role requires an APN and RN licensure with at least two years of APN experience in hospice or a clinical setting, plus strong knowledge of pharmacology and end-of-life care.

Qualifications

• Graduate of a Baccalaureate School of Nursing.
• Current State RN license and Advanced Practice Nurse (APN) license.
• Two years of APN experience in a clinical care setting, hospice or home health preferred.
• Comprehensive knowledge of pharmacology, pall­iative care, and end-of-life.

Responsibilities

• Consult with and educate the patient/family regarding disease process, self-care techniques, and end-of-life care.
• Consult with and assist the IDG in developing the patient/family plan of care to assure quality care.
• Interact with attending and consulting physicians regarding palliative care in collaboration with hospice physician.
• Inform physicians and other personnel of changes in the patient's needs and outcomes of intervention.
• Timely submission of all documentation.
• Performance of other duties as required.
• Ensure overall compliance with local, state and federal laws, Medicare regulations, and established personnel policies and procedures.
