Job summary

Elite Hospice, part of LHC Group, seeks an experienced Hospice Nurse Practitioner to provide palliative and supportive care consultations for hospice patients. You will work within the interdisciplinary team, coordinating plans of care, and reporting findings to the Hospice Medical Director.

Responsibilities include face-to-face visits, collaborating with physicians, documenting patient progress, educating families on care, and maintaining thorough medical notes.

Qualifications

• Current licensure as a Nurse Practitioner in the state of practice.
• Certification by a national body such as ANCC, NBCHPN, or AANP.
• 5+ years of nursing experience preferred.
• Current CPR certification.

Responsibilities

• Provide palliative and hospice care consultations
• Coordinate plan of care with Hospice Medical Director and interdisciplinary team
• Document patient status, goals, and interventions
• Educate patients and families on self-care and care options
• Maintain accurate clinical notes and report needs to leadership
