Agape Care Group in Virginia is seeking a Hospice Nurse Practitioner to provide compassionate hospice and palliative care to seriously ill patients, primarily in home settings. You will collaborate with a dedicated care team to align treatments with patients' goals and values.

The ideal candidate has 2 years of chronic disease management or long-term care experience, hospice background preferred, and reliable transportation.

Qualifications

• Heart to serve patients and families and a passion for providing the best possible care.
• Education: Graduate of an accredited Nursing program (Certificate, Diploma, ADN or BSN) preferred.
• Experience: 2 years in chronic disease management/long-term care within the last 5 years; hospice background preferred.
• Required: Reliable transportation and ability to lift 80–100 lbs.

Responsibilities

• Provide hospice and palliative care to seriously ill patients, supporting comfort and goals.
• Collaborate with the care team to align treatments with patient values and preferences.
