Job summary

VITAS Healthcare in Tinley Park, IL seeks an experienced Nurse Practitioner to join our hospice care team. The NP will assess patients, develop care plans, and coordinate with physicians to deliver high-quality palliative care.

You will prescribe or recommend treatments, monitor progress, and collaborate with hospice staff and volunteers as part of an interdisciplinary team. Illinois licensure and completion of an accredited NP program are required.

Qualifications

Minimum of 2 years hospice/home health experience in the past five years.
Completion of accredited Nurse Practitioner program is required.
State licensure is required.

Responsibilities

Records physical findings, and formulates plan and prognosis based on patient's condition.
Discusses case with physician and other health professionals to prepare comprehensive patient care plan.
Prescribes or recommends drugs or other forms of treatment.
Monitors and maintains progress of patients and reports progress to supervising physician.
Has the ability to work collaboratively with hospice employees and volunteers as part of an IDT.
