Job summary

VITAS Healthcare Corporation in Ocala, FL seeks a Nurse Practitioner to join the hospice care team. The role involves recording physical findings, formulating care plans, discussing cases with physicians, and prescribing or recommending drugs as part of an interdisciplinary team (IDT).

Qualified candidates will have a NP degree, state licensure, and at least 2 years of hospice, home health, or community health experience. The position may require lifting and assisting patients as needed.

• Requires completion of accredited Nurse Practitioner program.
• State licensure is also required.

Responsibilities

• Records physical findings, and formulates plan and prognosis, based on patient's condition.
• Discusses case with physician and other health professionals to prepare comprehensive patient care plan.
• Prescribes or recommends drugs or other forms of treatment.
• Monitors and maintains progress of patients and reports progress to supervising physician.
• Has the ability to work collaboratively with hospice employees and volunteers as part of an IDT.
