Job summary

Caring Hospice Services is seeking a part-time Registered Nurse for field hospice care across Union, Somerset, and Middlesex counties. The RN will provide nursing care focused on pain and symptom control, implement physician plans of care, and establish a nursing plan of care based on the nursing process.

Travel to patient homes and coordination with the hospice team are required. The role involves training and supporting families, ongoing patient assessments, and collaboration with prescribers

Qualifications

• Licensed RN in the state of New Jersey required.
• Strong assessment and care planning skills.
• Excellent communication with patients, families and hospice team.
• Proven ability to document care accurately and timely.

Caring Hospice Services -

The RN provides nursing care with special emphasis on control of pain and other symptoms to the hospice patient. Implements physician plan of care and establishes a nursing plan of care based upon nursing process. Trains, supervises and supports the family in provision of care to the patient.

Work area: Union, Somerset, Middlesex Counties

Job Functions for Registered Nurse:

• Assess the physical and psychological status of the patient.

• Reviews past and present medical history of the patient. Perceptive in planning and problem solving.

• Provides emotional support to caregivers.

• Communicates information regarding patients plan of care with other hospice staff members as needed.

• Performs ongoing assessment of patients conditions, physical needs and needs of family for support in giving care.

• Maintains ongoing communication with prescribing physician for changes in medication and treatments as needed.

• Licensed as a Registered Nurse in State of NJ.

• Knowledge of Medicare and Medicaid requirements outlined in the laws of licensure

• Ability to relate and treat terminally ill patients/families and their particular problems.

• Excellent organizational, interpersonal, telephone, documentation and communication skill.

• Access to reliable automobile with proof of liability insurance.
