Job summary

Health Link Home Health and Hospice is seeking a compassionate Licensed Vocational Nurse (LVN) to join our hospice team in a part-time role serving the East Bay. The LVN will collaborate with the Director of Nursing, interdisciplinary clinicians, and administrative staff to ensure patient-centered care and comfort for hospice patients.

The Hospice LVN provides skilled nursing care per individualized plans, monitors patient conditions, and ensures timely documentation.

Qualifications

• One year of nursing experience minimum.
• Hospice or Home Health experience preferred (not required).
• California Driver’s License and reliable transportation.
• Strong communication, organizational, and teamwork skills.
• Ability to work independently in field-based setting.

Responsibilities

• Provide nursing services within LVN scope of practice per plan of care.
• Implement treatments and monitor patient responses to care.
• Observe, document, and report changes to Hospice Clinical Director.
• Complete timely electronic documentation in Kinnser/WellSky using company devices.
• Follow agency policies, procedures, and regulatory guidelines.
• Support patient and family education as appropriate.
• Perform additional duties to support quality patient care.

Requirements
