Job summary

Health Link Home Health and Hospice Agency in San Francisco, CA, is seeking a Full-Time Home Health LVN to provide comprehensive in-home care. You will collaborate with the Director of Nursing, clinical staff, and office team to ensure coordinated, high-quality care for patients across San Francisco.

Qualifications

• Minimum 1 year of nursing experience in a medical setting.
• IV certification and experience with wound care.
• Home health experience recommended but not required.

Responsibilities

• Implement the plan of care initiated by the Registered Nurse
• Provide accurate and timely documentation consistent with the plan of care
• Assess and educate patients and caregivers regarding diagnosis, care plans, and treatments
• Participate in coordination of home health services, reporting additional needs to the RN or Clinical Supervisor
• Use equipment and supplies effectively and responsibly
• Complete online charting in Kinnser using company-provided devices
• Follow all company policies and procedures
• Perform other duties as assigned
