Job summary

Day City Hospice is seeking a compassionate Licensed Practical Nurse (LPN) to join our Dayton, OH team. You will provide direct patient care in a hospice setting, perform comprehensive assessments, and implement individualized plans to ensure comfort and dignity for patients at end of life.

The role involves administering pain management, educating families, documenting care in EMR, and collaborating with an interdisciplinary team to deliver coordinated, holistic care.

Qualifications

• Graduate of an accredited Practical/Vocational Nursing (LPN/LVN) program
• Current LPN/LVN license in the state of Ohio
• Minimum 3 years of nursing experience (hospice, home health, med-surg, or community health preferred)
• Current BLS certification
• Strong clinical, communication, and organizational skills
• Valid driver’s license, reliable transportation, and proof of insurance
• Understanding of hospice philosophy and end-of-life care

Responsibilities

• Provide direct patient care in accordance with the State Nurse Practice Act and hospice standards
• Conduct comprehensive nursing assessments and implement individualized plans of care
• Deliver skilled nursing services focused on comfort, dignity, and quality of life
• Administer and monitor pain and symptom management interventions
• Educate patients and families on disease progression, care plans, and symptom control
• Document patient care accurately and timely within electronic medical records (EMR)
• Collaborate with the interdisciplinary team to ensure coordinated, holistic care
• Evaluate patient responses and adjust care plans as needed

Location: Dayton, OH and surrounding areas

Schedule: 3x12-Hour Shifts/Week (Includes Every Other Weekend)

Schedule & Work-Life Balance

• Three 12-hour shifts per week
• Includes every other weekend rotation
