Day City Hospice in Dayton, OH is seeking a Licensed Practical Nurse (LPN) for a flexible, PRN/part-time hospice role. Schedule: three 12-hour night shifts per month in Dayton and surrounding areas.

Qualifications

• Graduate of an accredited Practical/Vocational Nursing (LPN/LVN) program.
• Current LPN/LVN license in the state of practice (Ohio).
• Minimum 3 years of nursing experience (hospice/home health preferred).
• Current BLS certification.
• Strong clinical assessment, communication, and organizational skills.
• Valid driver’s license, reliable transportation, and proof of insurance.
• Understanding of hospice philosophy and end-of-life care.

Responsibilities

• Provide direct patient care in accordance with State Nurse Practice Act and hospice standards.
• Perform comprehensive patient assessments and support individualized plans of care.
• Deliver skilled nursing services focused on comfort and quality of life.
• Administer and monitor pain and symptom management interventions.
• Educate patients and families on diagnoses, disease progression, and care plans.
• Document patient care accurately and timely in the EMR.
• Collaborate with the interdisciplinary care team to ensure coordinated hospice care.
• Evaluate patient response to treatment and adjust care as needed.
