Job summary

Mission Hospice & Home Care in the San Francisco Peninsula and South Bay seeks a dedicated Hospice Nurse Case Manager to assess, plan, and coordinate care for patients and families. The role emphasizes comfort, quality nursing assessments, and interdisciplinary collaboration.

The position is full-time with a Monday–Friday schedule, 8:30am–5:00pm, based in San Mateo. California RN licensure is required, with Public Health Nursing cert desirable and HPNA preferred, supporting compassionate

Qualifications

• One year of experience as a professional nurse in the past three years or a Bachelor’s degree in Nursing from an accredited School of Nursing.
• Current license as a registered professional nurse in the State of California.
• Public Health Nursing certification highly desirable.
• HPNA certification preferred.

Responsibilities

• Assume primary responsibility for a patient/family using the nursing process to assess, plan, implement and evaluate patient/family needs, goals, and interventions
• Coordinate all patient/family services and prioritization of needs with the members of the transdisciplinary team
• Use a case management approach and refer to other services as needed
• Coordinate the implementation of the plan of care and ensure care coordination and case management for each patient
• As applicable, apply specific criteria for admission and re-certification to hospice care to establish appropriate levels of care and patient’s eligibility

Education

Bachelor’s degree in Nursing

RN license in California

Public Health Nursing certification

The Hospice Nurse Case Manager is responsible for the assigned caseload or facility where they will be providing skilled nursing assessment, planning and care to maximize the comfort and health of patients and families.

Monday - Friday; 8:30am - 5:00pm

Service Area

San Mateo with additional coverage to San Ramon as needed

Mission Hospice & Home Care follows all CDPH vaccine requirements for healthcare personnel.

Pay Range

$58.00 - $73.65 hourly
