Mt. Washington Pediatric Hospital is seeking a Hospice Nurse Care Manager (RN) to deliver compassionate, patient-focused end-of-life care in the Omaha area.

This role emphasizes treating each patient like a loved one and developing tailored care plans. You will coordinate with families, facilities, and care partners, supervise aides as needed, and help team members improve through guidance and support, all while maintaining a low caseload to maximize comfort and dignity.

Our Hospice Nurse Care Manager (RN) Will:

• Provide individualized care by treating each patient as they would want a loved one treated.
• Develop care plans that reflect each person and their needs.
• Be detail-oriented, knowing that carefully managed patients (medications, treatments, communication, aide supervision, etc.) have more comfort and dignity.
• Focus on customer service when interacting with patients, families, communities/facilities, and all partners and co-workers.
• Help team members improve through instruction and support.
• Build and manage meaningful relationships with all partners.

Our Hospice Nurse Care Manager (RN) Has:

• An active state-required RN license or a compact state license.
• 2+ years of hospice experience.
• An active Driver's License with a reliable vehicle, insurance, and an acceptable driving record.
• The ability to meet the physical requirements for patient care with reasonable accommodations (e.g., Independently lift at least 30lbs).
• Humility to know that even experts learn something new every single day.
• A positive attitude and desire to make every day better for the people they encounter.
