Job summary

Metropolitan Jewish Health System (MJHS) seeks a Nurse Practitioner to join our Hospice and Palliative Care team. The NP provider supports consultations in nursing homes, hospitals and community settings across the Bronx, Upper Manhattan, and Isabella Nursing Home, delivering symptom management and goal-setting discussions.

You will work with MDs and NPs, develop care plans, educate staff and participate in on-call rotations.

Qualifications

• Master's degree in Nursing and graduation from an approved NP program is required.
• New York State RN licensure, and certification as Adult, Family, Gerontology, or Palliative Care NP is required.
• CPR-BLS and a NYS driver's license are preferred.

Responsibilities

• Staff palliative care consultation services with MDs and NPs.
• Develop care plans to prevent or mitigate illness burden for patients and families.
• Perform goal-setting discussions and hospice eligibility reviews.
• Support hospice operations through on-call rotation and face-to-face encounters as needed.
