Job summary

PruittHealth Hospice in South Carolina is seeking a PRN Nurse Practitioner to provide hospice and palliative care services in the patient’s home or care facility. You will perform comprehensive adult/geriatric assessments, evaluate and manage care plans, document promptly, and follow established policies to maintain continuity of patient care in a hospice setting.

Qualifications

• Must have active RN and NP licensure in state of practice.
• Board certification in adult, family, geriatric, or adult-gerontology primary care.

Responsibilities

• Proactive, collaborative team member in a hospice care environment.
• Conducts comprehensive adult/geriatric assessments of hospice patients and evaluates and manages plan of care.
• Commitment to caring for patients and partners.
• Complete all documentation and records in a timely and accurate manner.
• Follow established policies and procedures as related to the scope of practice.
• Maintain continuity of patient care in a hospice setting.
