Job summary
Affinity Health Management in Ohio is seeking a Music Therapist (board certified or eligible) to provide music therapy to address physical, emotional, cognitive and social needs of hospice patients. Reports to Clinical Manager. Responsibilities include developing care plans, directing therapy, preparing progress summaries, and collaborating with the interdisciplinary team. Requires degree in music therapy, state certification, CPR, and a valid driver's license.
Qualifications
Bachelor's or master's degree in music therapy approved by an accredited organization.
Certified to practice as a music therapist within the state.
At least 2 years of experience as a music therapist; hospice experience preferred.
Current CPR certification.
Licensed driver with automobile in good working order.
Responsibilities
Provide music therapy services to patients according to a written individualized plan of care.
Assess and evaluate therapeutic status and participate in the development of the total plan of care.
Direct music therapy treatment and determine priority needs for therapy.
Prepare and submit clinical and progress summaries.
Provide music therapy consultation to families/caregivers and in-service education for nursing staff.
Participate in interdisciplinary team meetings and quality improvement activities.
Engage in hospice orientation programs for team members.
