Job summary

Affinity Hospice Management LLC is seeking a Board Certified Music Therapist (or eligible to certified) to join our hospice team. The Music Therapist, contracted or employed, provides music therapy addressing physical, emotional, cognitive and social needs of patients.

The role reports to the Clinical Manager and requires a degree in music therapy, state certification, CPR, and a minimum of two years’ experience, preferably in hospice. A valid driver’s license is also required.

Qualifications

• Possesses a Bachelor’s or Master’s degree in music therapy from an accredited program.
• Certified to practice as a music therapist in the state.
• Minimum of 2 years of music therapy experience; hospice experience preferred.
• Maintains current CPR certification.
• Understands hospice philosophy and focus on comfort.

Responsibilities

• Provide music therapy services according to an individualized plan of care.
• Initiate music therapy programs and educate staff/family on goals.
• Prepare and submit clinical and progress summaries.
• Provide music therapy consultation to families/caregivers.
• Deliver in-service education to nursing staff as needed.
• Participate in interdisciplinary team meetings and QA activities.
• Contribute to orientation and onboarding of hospice team members.
