Job summary

Affinity Hospice Management LLC is seeking a licensed Music Therapist to provide individualized music therapy to patients in a hospice setting, contracted or employed. The role requires at least a Bachelor's or Master's in Music Therapy, state certification, and a minimum of two years' experience (hospice preferred).

A CPR certification and a valid driver's license are required to support on-site visits and home care services.

Qualifications

Bachelor's or Master's degree in music therapy from an accredited program.
State certification to practice music therapy.
Minimum 2 years of music therapy experience; hospice preferred.
Current CPR certification.
Possesses valid driver’s license and reliable vehicle.
Understanding hospice philosophy and comfort-focused care.

Responsibilities

Provide music therapy services to patients according to a written individualized plan of care.
Assess and evaluate therapeutic status and document progress.
Direct music therapy treatment and determine priority needs for therapy.
Report patient responses to physician and changes in condition.
Instruct staff and families in music therapy goals and program components.
Prepare and submit clinical and progress summaries.
Provide consultations to families and caregivers as appropriate.
Participate in interdisciplinary team meetings and care conferences.
Engage in orientation and quality improvement activities within hospice.
