Job summary

Affinity Health Management invites applications for a Board Certified Music Therapist (or eligible) to deliver patient-centered music therapy within hospice care. You will work under the Clinical Manager to address physical, emotional, cognitive and social needs through individualized plans of care.

Responsibilities include evaluating patients, directing therapy sessions, documenting progress, educating families and staff, and participating in interdisciplinary teams and quality initiatives.

Qualifications

• Requires a Bachelor’s or Master’s degree in Music Therapy approved by an accredited organization.
• Certified to practice as a music therapist in the state.
• At least 2 years of experience; hospice/community experience preferred.
• Current CPR certification.
• Licensed driver with insured vehicle per state requirements.

Responsibilities

• Provide music therapy services per a written plan of care.
• Assess and evaluate therapeutic status and help develop the care plan.
• Direct music therapy treatment with patients.
• Identify priority needs for music therapy.
• Report patient responses to physicians and adjust treatment as needed.
• Prepare and submit clinical/progress summaries.

Job Title/Position: Music Therapist

Reports To: Clinical Manager

Job Description Summary

The Music Therapist, contracted or employed, provides music therapy to address physical, emotional cognitive and social needs of patients.

Essential Job Functions/Responsibilities

• Provides music therapy services to patients according to a written individualized plan of care. This may include, but not be limited to:
• Assessing and evaluating therapeutic status, and participation in the development of the total plan of care. Evaluates home environment.
• Directing music therapy treatment.
• Determining priority needs for music therapy
• Reporting to physician patient’s reaction to treatment or changes in condition.
• Initiates music therapy program and instructs other personnel and/or family/caregivers in certain phases of music therapy with which they may work with a patient, as well as instructing them as to the goals of the music therapy program for the patient by participating in case conferences.
• Prepares and submits clinical and progress summaries based on the attainment of goals.
• Provides music therapy consultation to families/caregivers, as appropriate.
• Provides in-service education programs for nursing personnel as needed.
• Participates in the interdisciplinary group.
• Actively participates in the quality assessment performance improvement teams and activities.
• Participates in an orientation program for hospice team members.

Position Qualifications

• Possesses a baccalaureate or masters degree in music therapy approved by an accredited organization.
• Certified to practice as a music therapist within the state.
• At least (2) two years of appropriate experience as a music therapist. Community/Hospice experience is preferred.
• Possesses and maintains current CPR certification.
• Understands hospice philosophy, focus of care is comfort vs rehab
• Licensed driver with an automobile that is insured in accordance with state requirements and is in good working order.
