Job summary

Affinity Hospice Management LLC in Port Saint Lucie, FL is seeking a Board Certified Music Therapist (or eligible) to provide therapy addressing physical, emotional, cognitive and social needs of patients. The role may be contracted or employed and reports to the Clinical Manager.

Requires a degree in music therapy, state certification, CPR, and at least two years of experience; a licensed driver with a valid insured vehicle is also required.

Qualifications

• Possesses a baccalaureate or masters degree in music therapy approved by an accredited organization.
• Certified to practice as a music therapist within the state.
• At least 2 years of experience as a music therapist; hospice experience preferred.
• Current CPR certification.
• Understands hospice philosophy and focus on comfort.
• Licensed driver with an insured vehicle in good working order.

Responsibilities

• Provide music therapy services to patients per a written plan of care.
• Assess and contribute to the total plan of care, and monitor patient progress.
• Direct music therapy treatment and determine priority needs.
• Report patient reactions/condition changes to physicians.
• Initiate programs and educate staff and families on music therapy goals.
• Prepare clinical and progress summaries.
• Participate in interdisciplinary care teams and quality improvement activities.
• Join orientation programs for hospice team members.

JOB DESCRIPTION SUMMARY

The Music Therapist, contracted or employed, provides music therapy to address physical, emotional cognitive and social needs of patients.

ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES

1. 1. Provides music therapy services to patients according to a written individualized plan of care. This may include, but not be limited to:
   • a. Assessing and evaluating therapeutic status, and participation in the development of the total plan of care. Evaluates home environment.
   • b. Directing music therapy treatment.
   • c. Determining priority needs for music therapy
   • d. Reporting to physician patient’s reaction to treatment or changes in condition.
2. 2. Initiates music therapy program and instructs other personnel and/or family/caregivers in certain phases of music therapy with which they may work with a patient, as well as instructing them as to the goals of the music therapy program for the patient by participating in case conferences.
3. 3. Prepares and submits clinical and progress summaries based on the attainment of goals.
4. 4. Provides music therapy consultation to families/caregivers, as appropriate.
5. 5. Provides in-service education programs for nursing personnel as needed.
6. 6. Participates in the interdisciplinary group.
7. 7. Actively participates in the quality assessment performance improvement teams and activities.
8. 8. Participates in an orientation program for hospice team members.

POSITION QUALIFICATIONS

1. 1. Possesses a baccalaureate or masters degree in music therapy approved by an accredited organization.
2. 2. Certified to practice as a music therapist within the state.
3. 3. At least (2) two years of appropriate experience as a music therapist. Community/Hospice experience is preferred.
4. 4. Possesses and maintains current CPR certification.
