Job summary

AdvisaCare Home Health & Hospice is seeking a compassionate Medical Social Worker (MSW) in Michigan to support patients and families at life-limiting moments. You will provide one-on-one psychosocial care, coordinate with the hospice team, and participate in bereavement services.

The position offers flexible part-time/per diem hours, community-based visits, and meaningful work in the local area of Gaylord, Cheboygan, Charlevoix, Traverse City and surrounding regions.

Qualifications

• MSW degree from a CSWE-accredited program.
• One year of home health care experience preferred.
• Hospice experience preferred.
• Valid Driver's License, car insurance and reliable transportation.

Responsibilities

• Perform initial psychosocial, emotional, spiritual and bereavement assessments and assist in goal-directed care plans.
• Provide care under physician direction and participate in IDT conferences.
• Document all patient/family visits, calls and referrals in the patient record.
• Conduct reassessments of patient/family needs and provide counseling as required.
• Provide crisis intervention and individual or family counseling when indicated.
• Advocate for patients/family units and coordinate community resources for referrals.
• Maintain contact with community agencies to promote interagency cooperation and referrals.
• Offer ongoing counseling related to death and dying to patients and families as needed.
• Adhere to hospice policies.

We're looking for a compassionate, driven Medical Social Worker (MSW) who is passionate about enhancing the lives of patients with life-limiting illnesses.

Serving Gaylord, Cheboygan, Charlevoix, Traverse City and surrounding areas.

Requirements

• MSW degree from a school of social work accredited by the Council on Social Work Education (CSWE)
• One year of experience in Home Health Care preferred
• Hospice experience preferred
• Valid Driver's License, car insurance and reliable transportation

Job Duties

• Performs initial psychosocial, emotional, spiritual and bereavement assessments and assists in the development and implementation of goal-directed care plans
• Provides care under the direction of a physician. Participates in IDT conferences observing, assessing and bringing information regarding psychosocial, emotional, spiritual, physical and financial conditions affecting the patient and family
• Documents all patient/family visits, telephone contacts and referral actions timely, accurately and concisely in the patient record
• Conducts reassessments of patient/family needs and counseling as required
• Provides crisis intervention and individual or family counseling when indicated
• Plays an active role as a care advocate for the patient/family unit
• Develops and maintains contact with appropriate community agencies and services in order to promote interagency cooperation and to facilitate related referrals
• Provides ongoing counseling related to issues of death and dying to the patient and family as neededParticipates as a member of the Bereavement Team as assigned
• Adheres to all Hospice policies
