Job summary

AdvisaCare Home Health and Hospice in Michigan seeks a full-time MSW to join our compassionate team. You will perform psychosocial assessments, develop care plans, and support patients and families facing life-limiting illness in a hospice setting.

Candidates should have an MSW from a CSWE-accredited program; hospice or home health experience is preferred, and a valid driver’s license with reliable transportation is required.

Qualifications

• MSW degree from CSWE-accredited program.
• One year of Home Health Care experience preferred.
• Hospice experience preferred.
• Valid Driver's License and reliable transportation.

Responsibilities

• Performs psychosocial assessments and assists in care plan development.
• Participates in physician-directed care and IDT conferences.
• Documents visits and referrals timely and accurately.
• Conducts reassessments of patient/family needs and counseling as required.
• Provides crisis intervention and family counseling when indicated.
• Acts as a care advocate for patient/family unit.
• Maintains contact with community agencies to facilitate referrals.
• Provides ongoing counseling related to death and dying.
