Job summary

AdvisaCare Home Health & Hospice is seeking a compassionate Medical Social Worker (MSW) to join our patient‑centered team serving Gaylord, Cheboygan, Charlevoix, Traverse City and surrounding areas. You will assess psychosocial needs and support families through life‑changing moments in a one‑on‑one setting.

This role offers flexible part‑time/per diem opportunities, meaningful bereavement and counseling work, and collaborative care with physicians and an interdisciplinary team to improve

Qualifications

• CSWE-accredited MSW degree required.
• One year of Home Health Care experience preferred.
• Hospice experience preferred.
• Valid Driver's License and reliable transportation required.

Responsibilities

• Perform psychosocial, emotional, spiritual and bereavement assessments and help develop goal-directed care plans.
• Provide care under physician direction and participate in IDT conferences.
• Document all patient/family visits and actions timely in patient records.
• Conduct reassessments of patient/family needs and provide counseling as required.
• Provide crisis intervention and individual or family counseling as needed.

We're looking for a compassionate, driven Medical Social Worker (MSW) who is passionate about enhancing the lives of patients with life-limiting illnesses.

Serving Gaylord, Cheboygan, Charlevoix, Traverse City and surrounding areas.

Requirements

• MSW degree from a school of social work accredited by the Council on Social Work Education (CSWE)
• One year of experience in Home Health Care preferred
• Hospice experience preferred
• Valid Driver's License, car insurance and reliable transportation

Job Duties

• Performs initial psychosocial, emotional, spiritual and bereavement assessments and assists in the development and implementation of goal-directed care plans
• Provides care under the direction of a physician. Participates in IDT conferences observing, assessing and bringing information regarding psychosocial, emotional, spiritual, physical and financial conditions affecting the patient and family
• Documents all patient/family visits, telephone contacts and referral actions timely, accurately and concisely in the patient record
• Conducts reassessments of patient/family needs and counseling as required
• Provides crisis intervention and individual or family counseling when indicated
• Plays an active role as a care advocate for the patient/family unit
• Develops and maintains contact with appropriate community agencies and services in order to promote interagency cooperation and to facilitate related referrals
• Provides ongoing counseling related to issues of death and dying to the patient and family as neededParticipates as a member of the Bereavement Team as assigned
• Adheres to all Hospice policies
