Job summary

Residential Home Health and Hospice seeks a Medical Social Worker (MSW) to provide home-based hospice care in the Lake Orion area, working with an interdisciplinary team to support comfort, dignity, and quality of life.

The role involves psychosocial assessments, counseling, and developing care plans tailored to patient and family needs, with travel to homes within the territory. LIC/DRIVING is required.

Qualifications

• Master’s in Social Work from graduate program accredited by the Council on Social Work Education.
• 1–2 years post graduate social work experience in a health care setting.
• Must hold license as Medical Social Worker in the state where you work.

Responsibilities

• Deliver compassionate, patient-centered hospice care with an interdisciplinary team.
• Conduct psychosocial and bereavement assessments and develop care plans.
• Provide counseling and connect patients and families with resources.
• Ensure accurate EMR documentation and compliance with regulations.
• Travel to patient homes within assigned territory.
