Qualifications

• Master's degree in Social Work (MSW) required.
• Meets applicable hospice personnel qualifications and state requirements.
• 1+ year of MSW experience in a hospice setting.

Responsibilities

• Assess the psychosocial needs of patients and families related to terminal illness, end-of-life care, and home environment.
• Evaluate family dynamics, caregiver capabilities, communication patterns, and psychosocial risks.
• Provide counseling, emotional support, and crisis intervention to patients and caregivers as needed.
• Help patients and families navigate stress, anticipatory grief, changing family roles, and end-of-life concerns.
• Advocate for patient and family needs, dignity, and quality of life throughout the hospice journey.
