VitalCaring is hiring a Master’s Social Worker in Hospice to provide emotional, psychosocial, and practical support to patients and families facing terminal illness. You’ll work within an interdisciplinary team, guiding planning, resources, and connections throughout the hospice journey.

The role emphasizes compassion, patient-centered care, and autonomy in a field-based setting, with opportunities for ongoing training and professional development.

Qualifications

• Master’s degree in Social Work (MSW)
• 1+ year of MSW hospice experience
• Understanding hospice philosophy and end-of-life care
• Strong interpersonal skills with patients and families
• Valid driver’s license and reliable transportation for home visits

Responsibilities

• Assess psychosocial needs of patients and families related to terminal illness, coping, family dynamics, and home environment
• Provide counseling, emotional support, and guidance to patients and caregivers throughout the hospice journey
• Identify psychosocial risks and provide interventions and crisis support as needed
• Help patients and families navigate emotional and practical challenges of end‑of‑life care
• Provide culturally sensitive support respecting beliefs, traditions, values, and family dynamics
