Job summary

The Care Team Hospice in Reading, PA is seeking a Medical Social Worker (MSW) to provide emotional support, counseling, advocacy, and resource coordination for hospice patients and families. You will collaborate with the interdisciplinary team to ensure compassionate, patient-centered end-of-life care.

This role requires a Master’s in Social Work (MSW) preferred; BSW accepted with supervision. On-site at Reading with potential on-call rotation and mileage reimbursement.

Qualifications

• MSW preferred; BSW accepted with supervision.
• Experience in hospice, home health, or case management preferred.
• Knowledge of psychosocial assessment, crisis intervention, grief counseling, and community resources.
• Valid driver's license and reliable transportation.

Responsibilities

• Conduct psychosocial assessments.
• Provide counseling and emotional support to patients and families.
• Assist with advanced care planning and coordination of community resources.
• Participate in interdisciplinary team meetings and document services accurately in EMR HCHB.
