Job summary

The Care Team Hospice is seeking a compassionate Hospice Medical Social Worker (MSW) to provide psychosocial support, counseling, and advocacy for patients and families in Reading, PA and surrounding areas.

You will collaborate with an interdisciplinary team to coordinate community resources, support advance care planning, and ensure patient-centered end-of-life care. A Master's in Social Work is preferred, with supervision options for a BSW, and a valid driver's license for field duties.

Qualifications

• MSW from an accredited program preferred.
• BSW accepted with required supervision in accordance with state regulations.
• Experience in hospice, home health, healthcare, or case management preferred.

Responsibilities

• Conduct psychosocial assessments
• Provide counseling and emotional support to patients and families
• Assist with advanced care planning and coordination of community resources
• Participate in interdisciplinary team meetings
• Document services accurately and timely in the EMR system
