Job summary

The Care Team Hospice in Reading, PA seeks a Social Worker (MSW) to provide emotional support, counseling, advocacy, and coordination of community resources for hospice patients and families. You will collaborate with the interdisciplinary team to ensure compassionate, patient-centered end-of-life care.

Qualifications include an MSW (preferred) or a BSW with required supervision; experience in hospice or healthcare is a plus.

Hospice Medical Social Worker (Master's Level)

Join a compassionate and supportive hospice team dedicated to making a meaningful difference in the lives of patients and families. The Care Team Hospice is seeking a Social Worker (MSW) to provide emotional support, counseling, advocacy, and community resource coordination for hospice patients and their loved ones while collaborating closely with the interdisciplinary team to ensure compassionate, patient-centered end-of-life care.

Schedule, Hours & Territory

• Standard operating hours are Monday through Friday, 8:00 AM to 4:30 PM
• Reading, PA, and up to a 1-hour surrounding radius
• Participation in an on-call rotation may be required, with additional compensation provided

Qualifications

• Previous experience in hospice, home health, healthcare, or case management preferred.
• Knowledge of psychosocial assessment, crisis intervention, grief counseling, and community resources.

Field Role Requirements

• Valid driver's license, reliable transportation, and current automobile insurance.
• Ability to pass a criminal background check and Motor Vehicle Record (MVR) screening.
• Current TB testing in accordance with agency/State policy.
• Current CPR certification as required for the role.
