Residential Home Health and Hospice in North Huntingdon, PA is seeking an MSW for hospice care in patients' homes. You will collaborate with an interdisciplinary team to support comfort, dignity, and quality of life at end of life.

This field-based role offers autonomy, with leadership and education to grow your career. A Master’s in Social Work is required, with 1–2 years in health care, and a valid driver’s license for home visits.

Qualifications

• Master's in Social Work from graduate school accredited by the Council of Social Work
• 1–2 years post graduate social work experience in a health care setting
• Licensed MSW preferred
• Valid driver's license and ability to drive ~20% of day to patient locations

Responsibilities

• Deliver compassionate, patient-centered care in a hospice setting with the interdisciplinary team
• Conduct psychosocial and bereavement assessments and develop care plans
• Provide counseling and guidance to patients and families for end-of-life needs
• Apply critical thinking and ensure compliant EMR documentation
• Work independently but participate in team-based care
• Travel to patient homes within assigned territory
