Job summary

The Care Team Hospice in Bethlehem, PA is seeking a compassionate Licensed Medical Social Worker (MSW) to join our interdisciplinary team. In this role you will provide emotional support, counseling, crisis intervention, and resource coordination for patients and families navigating the hospice journey.

This part-time opportunity has the potential to grow to full-time as patient census grows. You will collaborate with physicians, nurses, chaplains, and social work colleagues, document

Qualifications

• Master's Degree in Social Work (MSW) from an accredited program preferred.
• Bachelor's Degree in Social Work (BSW) accepted with required supervision.
• Active Social Work license in Pennsylvania.
• Previous hospice/home health/case management experience preferred.
• Knowledge of psychosocial assessment and bereavement support.

Responsibilities

• Conduct comprehensive psychosocial assessments to identify patient and family needs and develop plans of care.
• Provide emotional support, counseling, crisis intervention, and guidance to patients, families, and caregivers.
• Assist with advance care planning, community resource coordination, financial assistance referrals, and bereavement support.
• Collaborate with physicians, nurses, chaplains, and the interdisciplinary team for coordinated care.
• Maintain accurate documentation in the electronic medical record (Homecare Homebase).
