Job summary

The Care Team Hospice seeks a compassionate Licensed Medical Social Worker (MSW) to join our team in Bethlehem, PA. The role focuses on psychosocial assessment, counseling, and resource coordination for patients and families navigating hospice care.

As part of an interdisciplinary team, you will support advance care planning, crisis intervention, and bereavement services, with documentation in Homecare Homebase. Part-time with potential to become full-time.

Qualifications

• Master's Degree in Social Work (MSW) preferred; state licensure.
• Active Social Work license in Pennsylvania
• Experience in hospice/home health/case management preferred
• Knowledge of psychosocial assessment, crisis intervention, grief counseling, and community resources

Responsibilities

• Conduct comprehensive psychosocial assessments and develop individualized care plans.
• Provide emotional support, counseling, crisis intervention, and guidance to patients, families, and caregivers.
• Assist with advance care planning, community resource coordination, and referrals for financial assistance and bereavement support.
• Collaborate with physicians, nurses, chaplains, and the interdisciplinary team to ensure coordinated, patient-centered care.
• Maintain accurate documentation in the electronic medical record (Homecare Homebase).

The Care Team Hospice is seeking a compassionate and dedicated Licensed Medical Social Worker (MSW) to join our growing team. This is an opportunity to make a meaningful difference by providing emotional support, counseling, advocacy, and resource coordination for patients and families navigating the hospice journey.

As a Hospice Medical Social Worker, you'll work closely with an experienced interdisciplinary team to address the emotional, psychosocial, and practical needs of patients and their loved ones. You'll help families navigate difficult decisions, connect them with valuable community resources, and ensure they receive the support they need every step of the way. As our patient census continues to grow, this part-time opportunity has the potential to become a full-time position.

Qualifications: Key Responsibilities

• Conduct comprehensive psychosocial assessments to identify patient and family needs while developing individualized plans of care.
• Provide emotional support, counseling, crisis intervention, and guidance to patients, families, and caregivers throughout the hospice journey.
• Assist with advance care planning, community resource coordination, financial assistance referrals, and bereavement support.
• Collaborate with physicians, nurses, chaplains, and the interdisciplinary team to ensure coordinated, patient-centered care.
• Maintain accurate, timely documentation of assessments, interventions, and care coordination activities within the electronic medical record (Homecare Homebase).

Qualifications

• Master's Degree in Social Work (MSW) from an accredited program preferred.
• Bachelor's Degree in Social Work (BSW) accepted with required supervision in accordance with state regulations.
• Active Social Work license in the state of Pennsylvania
• Previous experience in hospice, home health, healthcare, or case management preferred.
• Knowledge of psychosocial assessment, crisis intervention, grief counseling, and community resources.

Field Role Requirements

• Valid driver's license, reliable transportation, and current automobile insurance.
• Ability to pass a criminal background check and Motor Vehicle Record (MVR) screening.
• Current TB testing in accordance with agency/State policy.
• Current CPR certification as required for the role.
