Job summary

CommonSpirit Health at Home is seeking a Medical Social Worker - Hospice to support patients in home-based care. You will establish care plans, provide crisis intervention, and address social and economic factors impacting recovery.

Responsibilities include coordinating resources, educating patients and families, and ensuring timely documentation. A strong background in social work within healthcare and CPR/BLS certification are required.

Qualifications

• Master's degree from an accredited school approved by the Council on Social Work Education.
• Currently licensed to practice in the state served.
• One year social work experience in a health care setting.
• CPR/BLS certification through AHA or ARC and state compliance.

Responsibilities

• Establishing a care plan in accordance with the physician's plan of treatment.
• Providing crisis intervention as required.
• Assisting in understanding social, emotional and economic factors affecting patient improvement.
• Providing patient care education to patients and caregivers.
• Coordinating community referrals for public assistance, housing, and transportation.
• Collaborating with caregivers to maintain an optimum caring relationship.
• Completing and submitting required documentation accurately and timely.
