Job summary

Affinity Health Management is seeking a Hospice Medical Director to provide overall medical direction and leadership for the hospice program in the community. The role will oversee physician services, ensure regulatory compliance, and champion education for physicians and staff.

The candidate will work with the interdisciplinary group to maintain high-quality patient care, participate in QAPI initiatives, and coordinate care plan updates with attending physicians and the team.

Qualifications

• Licensed as a Doctor of Medicine or Osteopathy in the state without restriction.
• Maintains DEA registration and any controlled substance authorizations.
• Experience in hospice or palliative care; board certification preferred.
• Engages in ongoing medical education related to hospice care.
• Not excluded from Medicare/Medicaid or federally funded programs.
• Minimum of one year health care experience; hospice experience preferred.
• Demonstrates strong verbal and written communication and organization skills.
• Licensed driver with insured vehicle per state/organization requirements.

Responsibilities

• Provide overall medical direction to the hospice program.
• Ensure compliance with Medicare Conditions of Participation and applicable regulations.
• Participate in policy development with the interdisciplinary group.
• Review and update the plan of care with the team every 15 days.
• Support quality of care with QAPI and performance improvement projects.
• Act as medical resource to the IDG and attend meetings.
• Ensure physician coverage and availability for patients.
