Job summary
The Licensed Practical/Vocational Nurse is sought for per diem Hospice care in Modesto, providing direct patient care under RN supervision and supporting end-of-life care. Candidates must be a CA-licensed LPN/LVN with at least 1 year hospice experience; CPR certification preferred; and a valid driver's license.

Qualifications
- Graduate of an accredited practical nurse or vocational nursing program.
- Three years nursing experience; hospice or medical/surgical preferred.
- Current California LPN/LVN license.
- CPR certification current.
- Reliable transportation and valid driver's license.
- Good verbal and written communication skills.
- Understanding of hospice care philosophy and terminal patient needs.
Responsibilities
- Provide direct patient care per plan of care and end-of-life care.
- Document patient services accurately and timely.
- Educate patient and family about diagnosis and care plan.
- Coordinate hospice services with interdisciplinary team.
- Adhere to aseptic technique and use equipment properly.
- Support quality improvement initiatives and ongoing growth.
- Perform other duties as assigned by the registered nurse.
