Job summary

Compassionate Care is seeking a weekend Hospice LVN based in Modesto to provide direct patient care under RN supervision, following the plan of care and ensuring quality outcomes. The role requires 1 year of hospice experience, California LPN/LVN licensure, and CPR; per diem weekend scheduling offers flexibility.

The successful candidate will work within a supportive team, coordinate with interdisciplinary staff, and participate in ongoing quality initiatives to improve patient comfort and care

Qualifications

• Graduate of an accredited practical nurse or vocational nursing program.
• Three years nursing experience; hospice or medical/surgical preferred.
• Currently licensed as an LPN/LVN in California.
• CPR certification and a valid driver’s license with insured vehicle.
• Excellent verbal and written communication, and strong organizational skills.

Responsibilities

• Provide direct patient care under RN supervision, including end-of-life care duties.
• Follow the plan of care and document services accurately.
• Educate patients and families about diagnoses and self-care.
• Coordinate hospice services with interdisciplinary team.
• Use and prepare equipment, adhering to aseptic technique.
• Manage pain and symptoms and assess treatment responses.
• Participate in quality improvement activities.
• Perform other duties as assigned by the RN.
