Affinity Hospice Care Inc. in California looks for a Licensed Practical/Vocational Nurse to help patients heal and stay safe at home on a per diem basis. You will deliver direct care, follow personalized care plans, educate families, and document findings with a patient-first approach.

Join a supportive, family-oriented team; travel to patient homes is required. CA LPN/LVN license, CPR, and 3+ years of nursing experience are essential, with full-time conversion opportunities based on performance.

Qualifications

• Graduate of an accredited practical or vocational nursing program.
• Active LPN or LVN license in California.
• 3+ years of nursing experience; home health or med/surg background preferred.
• Current CPR certification with AHA.
• Valid driver’s license, reliable insured vehicle, and ability to travel to patient homes.
• Strong communication skills and a collaborative, patient-first mindset.

Responsabilités

• Provide direct patient care in accordance with the state Nurse Practice Act.
• Implement and follow individualized care plans established by the RN.
• Educate patients and their families on self-care techniques and diagnosis-related information.
• Document care accurately and in a timely manner.
• Collaborate with the care team — including OT, PT, HHA, and other disciplines — and communicate patient needs to the RN or Clinical Supervisor.
• Use supplies and equipment safely, adhering to aseptic technique.
• Perform other duties as required for patient services.
