Job summary

Affinity Hospice Care Inc. is seeking a Licensed Practical/Vocational Nurse for a per diem home hospice role in California. You will provide direct patient care, educate families, and implement care plans under the RN, visiting patients in their homes.

The position requires an active CA LPN/LVN license, CPR certification, and reliable transportation. Travel to patient homes is expected, with a focus on compassionate, patient-centered care.

Qualifications

• Graduate of an accredited practical or vocational nursing program.
• Active LPN or LVN license in California.
• 3+ years of nursing experience; home health or med/surg background preferred.
• Current CPR certification with AHA.
• Valid driver's license and reliable insured vehicle, and ability to travel to patient homes.
• Strong communication skills and a collaborative, patient-first mindset.

Responsibilities

• Provide direct patient care in accordance with the state Nurse Practice Act.
• Implement and follow individualized care plans established by the RN.
• Educate patients and their families on self-care techniques and diagnosis-related information.
• Document care accurately and in a timely manner.
• Collaborate with the care team — including OT, PT, HHA, and other disciplines — and communicate patient needs to the RN or Clinical Supervisor.
• Use supplies and equipment safely, adhering to aseptic technique.
• Perform other duties as required for patient services.
