Job summary

Uvalde Memorial Hospital’s Hospice team seeks a compassionate Licensed Vocational Nurse (LVN) to provide skilled nursing care in patients’ homes across the service area. You’ll work under an RN or physician, following the plan of care to support comfort, dignity, and quality of life during end-of-life care.

Responsibilities include delivering palliative nursing, symptom management, documentation, and collaboration with the interdisciplinary team. Travel is required, and on-call rotation applies.

Qualifications

• Current Texas LVN license is required.
• BLS for Healthcare Providers certification required.
• Valid driver’s license, insurance, and reliable transportation.
• One year LVN experience preferred.
• Hospice experience preferred.

Responsibilities

• Provide palliative nursing care in patients’ homes per the plan of care.
• Monitor patients’ conditions and assist with pain and symptom management.
• Report changes to supervising RN.
• Support physical, emotional, and psychosocial needs of patients and families.
• Communicate with RN Case Managers after visits to support continuity of care.
• Collaborate with physicians, nurses, and the hospice team.
• Maintain accurate, timely patient care documentation.
• Participate in interdisciplinary meetings and ongoing education.
• Contribute to a safe, respectful home environment.
• Participate in on-call responsibilities.
