Job summary

VNA Health is seeking a Licensed Vocational Nurse (LVN) for hospice care in Santa Barbara, CA. The LVN will provide direct patient care under the supervision of a Registered Nurse and contribute to patient and family education and care planning.

Responsibilities include following care plans, accurate documentation, symptom management, and coordination with the hospice team. A California LVN license and CPR certification are required; hospice experience is preferred.

Qualifications

• California LVN license active and current
• Current CPR certification
• Minimum one year experience; hospice or medical/surgical preferred
• Licensed driver with insured vehicle
• Demonstrates good verbal and written communication

Responsibilities

• Provide direct patient care under RN supervision
• Follow plan of care and approved care plans
• Document patient services accurately and timely
• Educate patients and families about diagnosis and self-care
• Coordinate hospice services with interdisciplinary team
• Manage pain and symptom relief and monitor responses
• Ensure compliance with state and regulations for hospice care
• Participate in quality improvement activities
