The role requires an active LPN license, nursing education, and the ability to travel locally. Flexible scheduling supports per diem coverage with weekend rotations and day shifts, delivering essential care when it matters most.

Qualifikationen

• Active LPN license in the state of employment.
• Graduate of an accredited nursing program.
• Clinical experience in hospice/home health or related setting preferred.
• Flexibility and adaptability across varied patient assignments.
• Strong assessment and communication skills.

Aufgaben

• Deliver professional nursing care within scope of practice.
• Conduct patient and family assessments and contribute to care plans.
• Complete visit types to meet patient and family needs.
• Administer medications and treatments per orders.
• Record clinical notes and outcomes.
• Communicate changes in patient condition to physicians and obtain orders.
• Coordinate with community health partners for care transitions.
