Addus HomeCare in Columbus, OH seeks a dedicated Licensed Practical Nurse to provide direct patient care in home health/hospice settings. You will implement nursing plans of care, document services accurately, and educate patients and families about diagnoses and self-care.

Qualifications

• Graduate of an accredited practical nurse or vocational nursing program with current license to practice in the state.
• Prior home health or hospice experience preferred with minimum of 1 year medical/surgical experience.
• Maintains a current CPR certification.
• Must be a licensed driver with an automobile that is insured and in good working order.

Responsibilities

• Provides direct patient care as defined in State Nurse Practice Act.
• Implements current nursing practice following a comprehensive assessment and the plan of care.
• Provide accurate and timely documentation of patient services to reflect the plan of care.
• Assess and provide patient and family education and information pertinent to diagnosis and self-plan of care.
• Participates in coordination of hospice services and reports needs to the interdisciplinary group.
• Provides appropriate pain/symptom management and evaluates response to treatments.
