Elara Caring in Dallas, TX is seeking a Licensed Vocational Nurse (LVN) to join our home health team delivering compassionate, patient-centered care in the patient’s home. You’ll follow care plans, monitor progress, and collaborate with the RN Case Manager to adapt plans as needs change.

Required: active LVN license, 1 year in a clinical setting, ability to lift up to 50 pounds, and reliable transportation for travel between patient homes. Hospice or home health experience is preferred.

• Must have a valid driver’s license or reliable transportation to travel between patient homes.

Job Description:

• Help prevent avoidable hospitalizations and emergency room visits through proactive patient care.
• Deliver compassionate, high-quality care that supports positive patient outcomes.

What is Required?

• Current State License as a Licensed Vocational / Practical Nurse Graduate of an accredited Certificate, Diploma, or Associate School of Nursing
• 1 year of experience in a clinical care setting
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
• Experience in a hospice or home health environment is preferred
