Job summary

Quality Home Health, Hospice & Palliative Care is seeking a compassionate LPN to join our hospice team in Utah. You will provide skilled nursing visits in patients' homes and facilities, following an established plan of care with RN supervision.

Mileage reimbursement is provided. The role emphasizes helping patients stay comfortable, supporting families, and delivering thorough documentation and coordination with the care team.

Qualifications

• Current LPN license in Utah.
• Graduate of an approved practical nursing program.
• Nursing experience preferred; hospice/home health/geriatric experience is a plus.

Responsibilities

• Provide skilled nursing visits to hospice patients as assigned and per plan of care.
• Perform nursing treatments and procedures within LPN scope under RN supervision.
• Observe patients for changes and communicate concerns promptly to the RN and care team.
• Provide medication, symptom-management, safety, and care education to patients and caregivers.
• Teach families appropriate patient care and self-care techniques.
• Complete accurate clinical documentation following each visit.
• Communicate with RN Case Managers, physicians, facilities, and caregivers.
• Coordinate with the care team to address patient needs promptly.
• Participate in interdisciplinary team meetings and care coordination as needed.
• Follow hospice policies, procedures, and individualized plan of care.
• Ensure compassionate, respectful, and responsive care for patients and families.
