Responsibilities

• Provide Hospice nursing care in patients' homes or care settings.
• Assess and deliver services per the Hospice plan of care.
• Maintain patient records per policy and regulatory guidelines.
• Supervise Home Health Aides and collaborate with IDT.
• Participate in on-call and after-hours coverage as required.
